Oscar winning actor Philip Seymour Hoffman, 46, was found dead this morning in his NYC West Village apartment of an apparent heroin overdose.  He reportedly rented the two-bedroom penthouse unit three months ago.

Screenwriter David Katz and another friend discovered the actor's body on a bathroom floor at 11:30 a.m. this morning and telephoned 911.  They reportedly stopped to check on him because he had not shown up as planned to pick up his kids, who were living at a nearby residence with their mother.

Upon arriving at the scene, authorities found the deceased actor with a needle still in his arm.  Investigators have gathered at the Greenwich apartment where police sources indicate two bags of heroin were found at the scene.

Hoffman struggled in the past with drug problems and suffered a relapse nearly two years ago after 23-years of sobriety.  He checked into a rehab facility in May 2013 for a short stint, citing the need to detox after returning to heroin.

In 2005, Hoffman played the title role in 'Capote,' for which he won multiple acting awards including an Academy Award for Best Actor. He received three more nominations for his supporting work in 'Charlie Wilson's War' (2007), 'Doubt' (2008) and 'The Master '(2012).

He also had the recurring role in the 'Hunger Games' franchise, portraying Plutarch Heavensbee.  He last appeared on the red carpet for the premiere of the latest film, 'Catching Fire.'

Hoffman was also an accomplished theater actor and director, appearing in numerous stage productions. His performances in three Broadway plays led to three Tony Award nominations: two for Best Leading Actor in 'True West,' and 'Death of a Salesman' and one for Best Featured Actor in 'Long Day's Journey into Night.'

The actor had been in a longtime relationship with costume designer Mimi O'Donnell. They met while working on the 1999 play 'In Arabia We'd All Be Kings,' which Hoffman directed. They have a son, Cooper Alexander and two daughters, Tallulah and Willa.

His family issued this statement: "We are devastated by the loss of our beloved Phil and appreciate the outpouring of love and support we have received from everyone.  This is a tragic and sudden loss and we ask that you respect our privacy during this time of grieving.  Please keep Phil in your thoughts and prayers."

cassandra lynn found dead in bathtub

Playboy playmate Cassandra Lynn Hensley was found dead in a bathtub at 9:00 a.m. Wednesday, Jan. 15, at the Los Angeles home of a friend in the Marina Del Rey area.  She was 34.

Police do not suspect foul play and their investigation suggests that the Utah native, who once lived in the Playboy mansion, died of a suspected drug overdose.  An autopsy was conducted on Thursday and toxicology results won't be released for several weeks, according to the L.A. County Coroner's office.

Hensley started modeling while studying to become an aesthetician and appeared in Playboy as Miss February 2006.  She said afterward, "I'm happy to be a Playmate.  It's a dream come true."

She talked about life after Playboy in a December 2013 interview with Philadelphia's Metropolis Nights magazine, "I have been a part of many great experiences.  Travel shows, magazine covers, and of course giving advice to young impressionable girls who think they need to act a certain way for a chance to be a model - classy not slutty."

She recently appeared in a photo spread as Angel of the Month and shared a photo on Twitter.

Cassandra Lynn was living in Fairfax, Virginia prior to her death and had been working as an event hostess.

One of her most recent social media tweets came last week, while she was staying with a pal in California, "#life dealing with one #jackass after another!!  I'm ready to go back."

phil everly dead at 74

Phil Everly, the younger of two Rock and Roll Hall of Fame siblings, died in Burbank, California on Friday, Jan. 3, of complications from chronic obstructive pulmonary disease.  He was 74.

His wife Patti told the Los Angeles Times that Phil was a lifetime smoker who "fought long and hard" after being diagnosed with Emphysema.  She acknowledged that the entire family is "absolutely heartbroken."

Phil and Don Everly gained fame as the Everly Brothers, with a string of hits in the 50's and 60's that included "Wake Up Little Susie,' 'Cathy's Clown,' 'Bye Bye Love,' 'Bird Dog,' 'Walk Right Back,' 'When Will I Be Loved' and 'All I Have to Do is Dream."

Everly was born to country stars Ike and Margaret Everly in Chicago in 1939.  He and his brother joined the family act as boys and went on to record their first hit 'Bye Bye Love' in 1957.

Phil's death comes just weeks after the release of Green Day star Billie Joe Armstrong and Norah Jones' tribute album, Foreverly.  It is a take on the brothers' second album Songs Our Daddy Taught Us.

Armstrong told USA Today  "They were so immaculate.  That record was pretty daring at the time.  A lot of other rock guys were trying to go pop.  The Everlys were playing torch songs and murder ballads.  For them to do something so dark and angelic was appealing to me."

Until his death, Phil Everly was involved with his own musical instrument accessories company.  Everly Music Company produces products designed for guitar and bass by Phil and Jason Everly, Phil's eldest son.

jeffrey pollack dead at 54

Hollywood director and producer Jeffrey Pollack collapsed and died while jogging on a running path in Hermosa Beach in Los Angeles on Monday.

Pollack, 54, was found unresponsive at 6:30 a.m. by another runner who told police that an iPod Nano that the dead man carried was still playing The Eagles when his body was discovered.

Investigators believe that he died of natural causes while exercising.  An autopsy and toxicology screening were undertaken by the L.A. County Coroner's Office and are not expected to be released for several weeks.

Jeffrey Ian Pollack co-created, wrote and produced the popular 90's sitcom 'The Fresh Prince of Bel-Air' for several seasons.  He also served as executive consultant on 'The Tyra Banks Show' and directed three feature films: 'Above the Rim,' 'Booty Call' starring Jamie Foxx, and 'Lost & Found.'

He and business partner Bennie Medina created a management-production company Handprint that boasted a number of high profile clients, including Jennifer Lopez, Mariah Carey and Tyra Banks.

Medina said on social media, "It is with a heavy heart that I must bid farewell to my dear friend, Jeff Pollack.  He was a talented writer, director, producer and entrepreneur ... but he would say his greatest accomplishment was his two children, Lael and Logan, to whom he was an amazing and devoted father."

Pollack exec produced two Billboard Music Awards in 1992 and 1993, and produced a TV documentary Jennifer Lopez in Concert.  Most recently, Jeff Pollack had been working with Jeff Beacher, who produces the Beacher's Madhouse variety shows at MGM Grand in Las Vegas.

Beacher said on Twitter, "RIP my partner, father, best friend and brother Jeff Pollack.  I can't put in words my emotions right now.  I love you buddy."

paul walker 1973 20133

Universal Studios released A Tribute to Paul Walker video on YouTube that includes scenes and clips of the actor who died in a fiery crash Saturday.  It has gone viral with 1.3 million views.

The 2:05 minute film is captioned "In Loving Memory" and features the late actor smiling and looking happy as the song 'I'm Coming Home' plays in the background.

Universal opted Monday to shut down production of 'Fast & Furious 7' in Atlanta for an unspecified period of time so the studio could assess the options for moving forward with the partially finished film starring Walker.

The family has requested donations be made to Walker's Reach Out Worldwide charity in lieu of flowers or other gifts.

Check it out below:

Positive identification and autopsies for Walker and his friend Roger Rodas were completed yesterday.  A preliminary report by the Los Angeles County Coroner's Office indicates Rodas, the driver, died of multiple traumatic injuries suffered upon impact.

Walker's cause of death was due to traumatic and thermal injuries -- suggesting that he was alive when fire consumed the vehicle several minutes after the initial impact.

According to a preliminary report released Wednesday, Dec. 4, by the Los Angeles County coroner's office, Paul Walker died of the combined effects of traumatic and thermal injuries.

The deaths of the 'Fast and Furious' star and his friend Roger Rodas have been ruled to be accidental.

Rodas, confirmed to be the driver with the aid of dental records, died of multiple traumatic injuries when the car hit a fixed object.

Toxicology results for both men are pending and could take six to eight weeks.

Video footage of the accident shows the car bursting into flames several minutes after the crash -- an indication that the actor was still alive when the red Porsche erupted into flames.  L.A. County detectives noted in the crash report that speed was a factor.

As a result of the untimely death of Walker, Universal Pictures has halted filming on 'Fast & Furious 7' for an unspecified period.  The latest installment of the popular franchise had been slated to be released in U.S. theaters on July 11, 2014.

The studio said in a statement, "At this time we feel it is our responsibility to shut down production for a period of time so we can assess all options available to move forward."

Walker is survived by a 15-year-old daughter Meadow, who was living with him at the time of his death.  Other survivors include his parents and three siblings.
